Once the request is approved, the medication will be sent directly to the requesting physician at no charge. Medication must be used within the scope of the labeled indications and by the patient for whom it was requested.

The medication will be shipped within 5 working days. Normally, a three-month supply will be shipped per approved request.

If additional medication is required, another original request must be submitted.
